They punch a hole in the side of the cow, and then put a plastic/rubber grommet into the hole to keep it open. It's called a fistula

It allows them to monitor what's in the stomachs (they have 4).

Although.. those look either recent, or badly done. I don't think they're supposed to leak like that.
